Job Description:
We are seeking a Licensed Practical Nurse (LPN) to join our team at Crystal Springs, a skilled nursing and rehabilitation facility under Autumn Lake Healthcare. The LPN will provide high-quality nursing care to residents in collaboration with the healthcare team, ensuring adherence to care plans and facility standards.

Responsibilities:

• Deliver direct patient care including administering medications and treatments as prescribed.

• Monitor and document patient conditions, reporting changes to the RN or physician.

• Assist residents with daily living activities and ensure their comfort and safety.

• Maintain accurate medical records and documentation in compliance with facility policies.

• Support interdisciplinary care planning and contribute to quality improvement initiatives.

• Provide emotional support to residents and families, promoting a positive care environment.
